Decreased B12 and/or folate levels may be associated with chronic disorders such as liver or kidney disease or alcoholism. In addition, certain medications, such as phenytoin (for seizures), metformin (for type 2 diabetes), or methotrexate (rheumatoid arthritis/cancer treatment), can result in deficiencies of these important vitamins. Incidence of vitamin B12 and/or folate deficiency is high among the elderly. Pregnant women require increased amounts of folate for proper fetal development. Women with a folate deficiency prior to pregnancy can expect the deficiency to be intensified during gestation, increasing the risk of premature birth and neural tube birth defects, such as spina bifida.
